Meet Alejo Davies-Jordan, REISA’s Patient Navigator!
Born and raised in Montreal, Alejo has worked in customer service, civil service, and healthcare as the go-to person for information and solutions. Always a good listener and invested in the user’s outcome, he helps overcome roadblocks, find ways forward, and ensure users find what they’re looking for.
A Concordia graduate in urban studies, he knows how to get around the city and is familiar with its many health and social service points. He’s seen Santé Québec from both sides, as a patient and as an employee, and has a knack for facilitating understanding and solutions.

No. The Patient Navigator Program is free and confidential.
Can the Patient Navigator give medical advice?
No. Patient Navigators do not provide medical advice or diagnoses. They help you understand your options, connect you with appropriate services, and guide you through the healthcare system.
Do I need a referral to use the program?
No. You can contact the Patient Navigator Program directly—no referral is required. Family members, caregivers, and healthcare professionals may also make referrals with your consent, where appropriate.
Can a family member or caregiver contact you on my behalf?
Yes. Family members, friends, and caregivers are welcome to contact us to discuss available services or request support, provided we obtain appropriate consent when necessary.
How long does it take to receive support?
Response times may vary depending on demand and the complexity of your request, but we strive to contact individuals as soon as possible after receiving an inquiry.
Is my information kept confidential?
Yes. Your personal information is handled confidentially and is only shared when necessary and with your consent, in accordance with applicable privacy regulations.
Do you book medical appointments for me?
Patient Navigators can help you understand how to access services and may assist you in connecting with providers or completing referrals. However, they do not typically book medical appointments directly unless this is part of the program’s scope.
Can you help me if I’m supporting someone else?
Yes. If you’re a caregiver, family member, or friend supporting someone who needs help accessing health or social services, we can provide information, guidance, and referrals to help you navigate available resources.
I‘m interseted in your services, but don’t live in your territory. Can I still access the Patient Navigator Program?
Our Patient Navigator Program serves individuals living within our designated territory. However, if you live outside our service area, you may still be able to access support through another Patient Navigator Program in your region.
