Patient Navigation

Navigating the health care system can feel overwhelming, especially for English-speaking community members in the East and North Territories of Montreal. That’s where our patient navigation program steps in. Our dedicated navigator serves as a trusted guide, ensuring that patients are never alone on their journey. We provide clear information, compassionate emotional support, and practical help every step of the way.

The patient navigation program supports English-speaking patients in the East and North Territories of Montreal as they navigate the health and social services network, offering information, emotional support, and logistical help.
The patient navigator connects patients to services, helps them access care, and supports communication with health care staff. Additionally, they work with health care workers and partners to improve access to care in English. The navigator offers emotional support, advice, a listening ear, and advocates for patients.

Meet our Patient Navigator

Meet Alejo Davies-Jordan, REISA’s Patient Navigator!

Born and raised in Montreal, Alejo has worked in customer service, civil service, and healthcare as the go-to person for information and solutions. Always a good listener and invested in the user’s outcome, he helps overcome roadblocks, find ways forward, and ensure users find what they’re looking for.

A Concordia graduate in urban studies, he knows how to get around the city and is familiar with its many health and social service points. He’s seen Santé Québec from both sides, as a patient and as an employee, and has a knack for facilitating understanding and solutions.

What is Patient Navigation?

Patient navigation helps those facing obstacles in interacting with the health care system. Whether it’s language, unfamiliarity with the health system, or the added complexity of geography and culture. Navigators step in to smooth the process, making it easier for patients to get the information and support they need to move forward with confidence in their care.

The Role of a Patient Navigator

Their role includes guiding patients through appointment logistics, assisting with medical forms, and ensuring that key health information is clearly communicated. Navigators also act as a bridge between patients and healthcare providers, offering accompaniment to appointments when necessary and providing emotional support to patients and their families.
